Scope
This notice covers the Omni Health Company website. Each Omni Health company publishes its own notices for its products and services. Patient information handled inside those products is governed by the agreements and notices for those products, including business associate agreements where HIPAA applies.
What we collect
When you send the contact form, we receive the details you enter: name, work email, organization, role, phone, state, the services you select, your message and the page you sent it from.
The website does not use advertising cookies or third-party analytics. Our hosting provider keeps standard server logs, such as IP address and request time, for security and operations.
How we use it
- To respond to your request and route it to the Omni Health company that covers it.
- To keep a record of the conversation.
- To protect the website against abuse.
We do not sell personal information and do not share it for advertising.
Where it is stored
Form submissions are stored in a database hosted in the United States with access limited to authorized staff. The website itself is hosted on Vercel.
Health information
Do not send patient health information through this form. It is not designed to receive protected health information.
Retention
We keep contact requests for as long as needed to respond and to maintain business records, then delete them.
Your choices
You can ask us to access, correct or delete the information you sent by submitting a request through the contact form and selecting your preference in the message.
Changes
We update this notice when our practices change and revise the date above.
